Community Health Specialist

We are seeking a highly motivated and enthusiastic registered nurse to join our innovative community nursing team at Leatherhead Primary Care Network.
As a Community Health Specialist, you will be responsible for assessing, delivering, and evaluating quality care to patients in their homes, care homes, and through in-reach in to hospitals.
You will work closely with the community hospitals and specialist teams to ensure a whole system approach to working across Surrey Downs.
Key Responsibilities- Provide ongoing care and support to patients in their homes and care homes
- Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources
- Reduce risk of admission for patients
- Support people in their homes, people in care homes, and through in-reach in to hospitals where necessary
- Work closely with the community hospitals and specialist teams
Surrey Downs Health and Care (SDHC) deliver care closer to people's own communities through our Primary Care Networks and our innovative partnership of local NHS organisations.
We have a track record of providing person-centred care that goes beyond organisational boundaries to do what is best for the individual.
Our partnership includes The three GP federations GP Health Partners, Dorking Health Care and Surrey Medical Network representing practices that operate in the Surrey Downs area, CSH Surrey, Epsom and St Helier's University Hospitals NHS Trust, and Surrey Council County.
Requirements- Registered Adult Nurse with NMC
- Valid Driving Licence (to drive within UK)
- Access to a vehicle for work purposes
- Ability to assess, deliver, and evaluate quality care
- Effective communicator
Please note that these roles require post-holders to hold a current and valid driving licence enabling them to drive in the UK and have access to a vehicle for work purposes.
