Community Health Specialist

We are seeking a highly skilled and experienced Community Health Specialist to join our team at Epsom and St Helier University Hospitals NHS Trust. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and support in the community.
Key Responsibilities- Provide ongoing care and support to patients in their homes, care homes, and through in-reach services in hospitals.
- Deploy the most appropriate staff member(s) in a coordinated and timely way to best use community resources.
- Reduce risk of admission for patients by providing proactive and preventative care.
- Work closely with community hospitals and specialist teams to ensure a whole system approach to healthcare.
- Registered Adult Nurse with NMC registration.
- Valid Driving Licence (to drive within the UK).
- Teaching qualification.
- Evidence of continued professional development post registration.
- Access to a vehicle for work purposes.
- Role model to others in both clinical and team cohesiveness.
- Evidence of experience of team management and multi-disciplinary working.
- Relevant post-registration experience.
