Job Overview

We are seeking a highly motivated and enthusiastic Registered Nurse to join our innovative community nursing team at Leatherhead Primary Care Network. As a Community Health Specialist, you will play a vital role in delivering high-quality care to patients in their own homes, community hospitals, and through in-reach into hospitals.

Main Responsibilities

As a key member of our team, you will be responsible for:

  • Assessing the care needs of patients and developing holistic care packages
  • Delivering high-quality care to patients in their own homes, community hospitals, and through in-reach into hospitals
  • Working closely with the community hospitals and specialist teams to ensure a whole-system approach to care
  • Participating in the development of innovative practice and contributing to team management

Requirements

To be successful in this role, you will need:

  • Registration with the Nursing and Midwifery Council (NMC)
