Health and Wellbeing Coach

We are seeking a highly skilled and compassionate Health and Wellbeing Coach to join our team at Railway Medical Group. As a key member of our Health and Wellbeing Team, you will play a vital role in supporting individuals to manage their physical and mental health, improve their quality of life, and achieve their health goals.
Key Responsibilities- Provide one-to-one and group health coaching support to individuals, empowering them to take control of their health and wellbeing.
- Work closely with GPs, practice nurses, and other healthcare professionals to ensure seamless care coordination and referrals.
- Develop and maintain strong relationships with local agencies and community groups to promote health coaching and raise awareness of its benefits.
- Collect and report data on service user experience and impact of health coaching, contributing to the evaluation and improvement of our services.
- Participate in regular supervision and professional development opportunities to enhance your skills and knowledge.
- Completed a four-day PCI accredited health coaching training course or be willing to complete one prior to taking referrals.
- Completed Tier 1 level of practice/expertise for health and wellbeing coaches or willing and supported to do so.
- GCSE Grade 4 (C) or above in Maths and English.
- Excellent communication and interpersonal skills, with the ability to build rapport with individuals from diverse backgrounds.
- Ability to work independently and as part of a team, with minimal supervision.
- Proficient in Microsoft Office and web-based services.
