Care Coordinator

Working within South Trafford Community Mental Health Services is an exciting and rewarding opportunity to make a positive impact on the lives of individuals and communities.

Main Duties of the JobTrafford Community Mental Health Services are a committed integrated service that strives to provide quality recovery-based support to people with severe and enduring mental health-related difficulties in Trafford 7 days a week, 365 days a year.
The service calls upon a wide range of evidence-based holistic assessments and interventions.
As a Community Psychiatric Nurse working in the service, you will be required to undertake the role of Care Coordinator in line with CPA process, working proactively and flexibly within a multi-disciplinary team model.
You will also be expected to provide clinical leadership, assist in the organizing, supporting, supervision, and development of junior staff members, and act as an assessor/mentor for student nurses.

Experience
Essential Criteria
- Registered Mental Health Nurse.
- At least 1 year post-registration experience.
Desirable Criteria
- Experience of mentoring and/or supervision.
- Previous experience of working in the community.
Knowledge
Essential Criteria
- Comprehensive knowledge of CPA, MHA, and Capacity Act.
Desirable Criteria
- Further qualifications relevant to the role.
