Job Overview

Working within South Trafford Community Mental Health Services is an exciting and rewarding opportunity to make a positive impact on the lives of individuals with severe and enduring mental health-related difficulties.

Main Duties of the Job

As a Community Psychiatric Nurse, you will be required to undertake the role of Care Coordinator in line with the Care Programme Approach (CPA) process, working proactively and flexibly within a multi-disciplinary team model. Your defined complex caseload may consist of individuals on the Assertive Outreach pathway, and you will be expected to provide clinical leadership and support to junior staff members.

Key Responsibilities
  • Undertake holistic assessments and interventions using evidence-based practices
  • Develop and implement individualized care plans in collaboration with service users and their families
  • Provide clinical leadership and supervision to junior staff members
  • Act as an assessor/mentor for student nurses
  • Contribute to the further development of the service and implementation of its Clinical and Social Care Governance agenda

Person Specification

We are seeking a Registered Mental Health Nurse with at least 1 year post-registration experience. Desirable criteria include experience of mentoring and/or supervision, as well as previous experience of working in the community. Comprehensive knowledge of CPA, MHA, and Capacity Act is essential.
