Care Coordinator

We are seeking a highly skilled and dedicated Care Coordinator to join our team at Greater Manchester Mental Health. As a key member of our community mental health services, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental health-related difficulties.
Main Duties and ResponsibilitiesAs a Care Coordinator, you will be responsible for:
- Undertaking comprehensive assessments and developing personalized care plans in collaboration with service users, families, and multidisciplinary teams.
- Providing proactive and flexible support to individuals with complex needs, including those on the Assertive Outreach pathway.
- Leading clinical leadership and mentoring junior staff members to ensure the delivery of high-quality care.
- Acting as an assessor and mentor for student nurses, promoting their professional development and growth.
- Contributing to the further development of our service and implementing its Clinical and Social Care Governance agenda.

We are seeking a candidate with:
- Registration as a Mental Health Nurse.
- At least 1 year post-registration experience.
- Comprehensive knowledge of CPA, MHA, and Capacity Act.
