Care Coordinator

We are seeking a highly skilled and dedicated Care Coordinator to join our team at Greater Manchester Mental Health. As a key member of our community mental health services, you will play a vital role in providing high-quality care and support to individuals with severe and enduring mental health-related difficulties.
Main Duties and ResponsibilitiesAs a Care Coordinator, you will be responsible for:
- Undertaking comprehensive assessments and developing personalized care plans in collaboration with service users, families, and multidisciplinary teams.
- Providing proactive and flexible support to individuals with complex needs, including those on the Assertive Outreach pathway.
- Leading clinical leadership and mentoring junior staff members to ensure the delivery of high-quality care.
- Acting as an assessor/mentor for student nurses and contributing to the development of the service.

Person SpecificationWe are looking for a Registered Mental Health Nurse with at least one year of post-registration experience. Desirable criteria include experience of mentoring and/or supervision, as well as previous experience of working in the community. Comprehensive knowledge of CPA, MHA, and Capacity Act is essential.
