Job Summary:

Chelmscare is seeking a highly skilled and experienced Area Community Manager to join our team. As a key member of our management team, you will be responsible for overseeing the delivery of high-quality care services to our clients.

Key Responsibilities:

  • Field Supervision: Provide ongoing supervision and support to our Personal Care Assistants to ensure they are meeting the individual needs of our clients.
  • Operational Support: Assist with the assessment, setup, and review of care packages to ensure they meet the needs of our clients.
  • Workforce Development: Ensure a skilled workforce is in place to deliver safe and quality care to our clients.
  • Client Relationships: Build and maintain positive relationships with clients, their families, and other professionals involved in their care.
  • Health and Safety: Conduct regular health and safety visits to ensure the well-being of our clients.
  • Care Plan Reviews: Conduct interim reviews of care plans and care packages to ensure they are meeting the needs of our clients.
  • Quality Assurance: Audit visit notes and EMAR charts to ensure high-quality care is being delivered.
  • Emergency Response: Respond promptly to emergencies and ensure the safety of our clients.

Requirements:

  • Experience in care services management
  • Strong leadership and supervisory skills
  • Ability to build and maintain positive relationships with clients and staff
  • Knowledge of care standards and regulations
  • Ability to work independently and as part of a team
