About the Role:

The successful candidate will have a strong awareness of the needs of individuals with disabilities or health problems. They will be able to work independently and as part of a small team, maintaining detailed and accurate electronic records.

Key Responsibilities:

  • Provide support and guidance to individuals with disabilities, ensuring they receive the assistance they need to achieve their goals.
  • Collaborate with colleagues to develop and implement strategies that promote accessibility and inclusion.
  • Maintain accurate and up-to-date records of client interactions and progress.
  • Travel to outreach venues throughout the area to provide support and services to individuals with disabilities.

Requirements:

  • Strong awareness of the needs of individuals with disabilities or health problems.
  • Ability to work independently and as part of a small team.
  • Excellent communication and interpersonal skills.
  • Ability to maintain detailed and accurate electronic records.
  • Own transport and willingness to travel to outreach venues.
