Position Overview

This is a permanent opportunity offering up to 30 hours of work per week.

Livewell Southwest stands as a prominent independent social enterprise in England, functioning within the NHS framework akin to general practitioners, dental professionals, and pharmacies. Our dedicated workforce of 3,000 provides comprehensive health and social care services to individuals in various regions.

Currently, we are seeking Community Support Specialists to join our Integrated Learning Disability Team.

In this role, professionals will engage with individuals facing intricate challenges. These challenges are characterized by unpredictability or ambiguity, where individuals may exhibit resistance or pose significant risks to themselves. The nature of these situations often requires considerable time and effort due to the multifaceted factors contributing to social exclusion. Decision-making necessitates careful consideration of diverse perspectives, including psychological, biological, legal, environmental, and economic factors.

Livewell Southwest is dedicated to fostering an integrated workforce in health and social care, adhering to the fundamental principles of the social work profession.

Key Responsibilities

The selected candidate will be responsible for executing statutory social care duties on behalf of the organization while upholding the standards outlined in the Professional Capabilities Framework for Social Work. This includes conducting thorough and proportionate assessments based on evidence, such as Mental Capacity assessments and Care Act evaluations for individuals with varying needs that may pose significant risks to their well-being. Additionally, the role involves coordinating and reviewing the provision of social care support.

Whenever feasible, the candidate will encourage individuals to embrace risks in complex scenarios to promote their desired outcomes and aspirations. The role also entails conducting safeguarding inquiries and making decisions in the best interest of individuals, which may involve considerations that could restrict their liberty.

Work Schedule

All staff members at Livewell Southwest are expected to be adaptable and willing to contribute to a service that operates seven days a week.

Essential Qualifications

The ideal candidate will possess a qualification in Social Work, such as a postgraduate diploma, bachelor's degree, or master's level.

Experience Requirements

Proven experience in the health and social care sector is essential, along with knowledge of social work theories and models, their strengths and limitations, and their practical applications.

Familiarity with the legal framework relevant to social work, including the Care Act (2014), Mental Capacity Act (2005), Mental Health Act, and the Human Rights Act is crucial.

Knowledge and Skills

Strong communication skills are vital for maintaining positive relationships with colleagues, service users, and their families. The candidate should be able to navigate sensitive issues effectively and present evidence-based decisions in complex situations.

Additionally, the role requires participation in risk management processes and collaboration with colleagues to ensure high-quality care delivery.
