Position Overview

As a vital member of the healthcare team, the Medication Management Specialist plays a crucial role in enhancing patient care within the Scarborough Medical Group. This position is designed to optimize medication use, ensuring safe and effective prescribing practices.

Key Responsibilities

  • Collaborate with Clinical Pharmacists, Community Pharmacists, and healthcare professionals to adhere to established prescribing protocols.
  • Process medication refill requests, including transitioning acute prescriptions to ongoing treatments and adjusting medication quantities as necessary.
  • Issue prescriptions in accordance with clinical guidelines and within the scope of practice.
  • Maintain regular communication with community and hospital pharmacists to ensure continuity of care.
  • Provide patients with guidance on medication optimization and proper inhaler techniques.
  • Conduct medication review clinics to assess and improve patient medication regimens.
  • Support the practice in achieving its prescribing quality targets.
  • Address medication-related inquiries, including urgent requests and patient prescription questions.
  • Ensure accurate documentation of patient medication records within the clinical system, including advice provided and actions taken.
  • Coordinate with secondary and primary care teams to ensure appropriate medication management during transitions of care.
