About the Role
Pennine Care NHS Foundation Trust is seeking a dedicated and experienced Senior Mental Health Practitioner to join our dynamic Mental Health Access Team in Stockport. In this vital role, you will be responsible for providing comprehensive assessments and coordinating care for individuals experiencing mental health challenges.
Your Responsibilities
- Conduct thorough assessments and risk evaluations to determine the needs of service users.
- Coordinate assessment clinics, prioritizing appointments to ensure timely access to care.
- Develop individualized care plans in collaboration with service users, families, and other healthcare professionals.
- Provide mentorship and guidance to junior staff members, fostering a supportive and collaborative work environment.
About You
You will be a highly motivated and compassionate individual with a strong commitment to providing high-quality mental health care. Your expertise in risk assessment, treatment interventions, and crisis intervention will be essential in this role.
Essential Requirements
- Registered Mental Nurse (RMN), Registered Nurse for Mental Health (RNMH), or equivalent Social Work qualification.
- Completion of relevant post-registration qualifications and a commitment to continuous professional development.
- Extensive post-registration experience working with individuals experiencing complex mental health needs.
- Proven ability to triage referrals effectively and coordinate care across various healthcare settings.

- Strong communication and interpersonal skills, enabling effective collaboration with service users, families, and colleagues.
