Job Description

Job Title: Senior Mental Health Practitioner

Job Summary:

We are seeking an experienced and skilled Senior Mental Health Practitioner to join our Home Treatment Team. The successful candidate will provide high-quality care and support to individuals with mental health needs, working closely with our multidisciplinary team to deliver effective interventions and promote recovery.

Key Responsibilities:

  • Provide comprehensive assessments and risk formulations for service users
  • Develop and implement individualized care plans to promote recovery and well-being
  • Work collaboratively with colleagues to deliver intensive home treatment and crisis interventions
  • Supervise and support junior colleagues to ensure high-quality care and service delivery
  • Contribute to the development and implementation of service policies and procedures

Requirements:

  • Registered Mental Health Nurse (RMN), Social Worker, or Occupational Therapist (OT) with a strong background in mental health care
  • Proven experience in home treatment and crisis interventions
  • Excellent assessment and risk management skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Strong communication and interpersonal skills
