About the Role

We are seeking a highly skilled and motivated Mental Health Practitioner to join our Older Peoples Mental Health Liaison Team in Stockport. As a key member of our team, you will work closely with the Urgent Community Response Team to provide specialist mental health nursing advice and joint assessments to individuals in crisis.

Main Responsibilities
  • Represent the Mental Health Liaison Service while working with the Urgent Community Response Team in Stockport.
  • Provide direct care to service users, including risk assessments, psychological needs, and cognitive functioning.
  • Assess and manage clients presenting with co-morbidities of physical and mental health needs, including the impact of medication and infection.
  • Contribute to making decisions regarding client care in consultation with Intermediate Tier multi-disciplinary teams.
  • Undertake short-term nursing interventions and/or coordinate transition of care to other specialist support.
Requirements
  • Registered Mental Health Nurse (RMN) qualification.
  • Significant post-registration experience at Band 5 or above.
  • Experience of working within older people's mental health inpatient, community, or liaison settings.
  • Experience of implementing programs to manage Behavioral and Psychological Symptoms of Dementia (BPSD).
