About the Role

This is an exciting opportunity for mental health professionals to join our team at Pennine Care NHS FT as a Mental Health Liaison Practitioner. As a key member of our Psychological Medicine Service, you will have the opportunity to work alongside local health and social care providers to develop innovative services that have received national recognition.

Main Responsibilities
  • Carry a relatively small caseload and work closely with other healthcare professionals to provide effective joined-up management of patient care through education, consultation, assessment, and joint working of cases.
  • Act as a representative of mental health services within an identified neighbourhood multi-disciplinary team, providing specialist advice and support in relation to colleague interactions with patients.
  • Advise on suitability of new referrals, assessments, and offer interventions for adults with complex mental health presentations and complex comorbid mental and physical health.

Person Specification
  • Education / Qualifications:
    • RMN / Social Worker / OT
    • Demonstrable post-qualification experience in mental health
    • Willingness to undertake further training relevant to the practitioner role
    • Accredited or registered relevant professional body
    • Short courses and experience to postgraduate diploma level
    • Specialist knowledge of complex comorbid physical and mental health issues
  • Knowledge:
    • Experience in working with people with complex and severe mental health needs
    • Extensive knowledge/experience in social inclusion and recovery model of care
    • Knowledge of mental health legislation
  • Experience:
    • Experience of supervision
    • Experience of working in a community or primary care setting
    • Experience of MDT working
    • Experience of working with people with co-morbid physical symptoms and mental health
  • Skills and Abilities:
    • Ability to undertake comprehensive assessments of need and risk for patients with multiple/complex issues
    • Ability to formulate care plans and risk management plans
    • Ability to work effectively as part of a MDT
    • Ability to supervise or advise practice and primary care staff
    • Effective time management
    • Excellent written and verbal communication skills
    • Recording relevant documentation to a professional standard
