About the Role

We are seeking a highly skilled and experienced Mental Health Liaison Practitioner to join our team at Pennine Care NHS FT. As a key member of our Psychological Medicine Service, you will play a vital role in improving the psychological care of people with complex physical and mental health needs.

Job Summary

The successful candidate will be responsible for providing specialist advice and support to healthcare professionals and colleagues, working closely with patients with persistent physical symptoms and complex mental health needs. You will be part of an innovative team leading service developments alongside local health and social care providers.

Main Duties
  • Carry a relatively small caseload and liaise closely with other healthcare professionals to provide effective joined-up management of patient care.
  • Act as a representative of mental health services within an identified neighbourhood multi-disciplinary team, providing specialist advice and support.
  • Advise on suitability of new referrals, assessments, and offer interventions for adults with complex mental health presentations and complex comorbid mental and physical health.

Person Specification

The ideal candidate will have:

  • RMN, Social Worker, or OT qualification.
  • Demonstrable post-qualification experience in mental health.
  • Ability to undertake comprehensive assessments of need and risk for patients with multiple/complex issues.
  • Ability to formulate care plans and risk management plans.
