Position Overview

We are seeking a dedicated and skilled Senior Rehabilitation Specialist with extensive expertise in the areas of frailty and geriatric care to join our innovative integrated Frailty Virtual Ward team. This pioneering service is designed to enhance collaboration between acute and community healthcare providers, focusing on delivering exceptional care to patients in their own homes within the diverse communities we serve.

Key Responsibilities

- Deliver a high-standard Physiotherapy service to patients enrolled in the Frailty Virtual Ward, primarily within their residential settings, while promoting and maximizing patient potential within established service parameters.

- Provide expert guidance, support, and education to other members of the Multidisciplinary Team (MDT) regarding your specialized area of practice.

- Collaborate with the Service Manager to lead your team in the planning, coordination, delivery, and evaluation of the Physiotherapy services offered to patients in your designated area.
