Job Overview

We are seeking a dedicated and proactive Community Rehabilitation Physiotherapist to join our Integrated Care Team. This role is designed for individuals with a keen interest in enhancing physical health within the community. The Integrated Care Team operates as a cohesive unit, collaborating closely with District Nurses, physiotherapists, and occupational therapists to ensure a comprehensive and holistic approach to patient care.

Main Responsibilities

As a Community Rehabilitation Physiotherapist, you will:

  • Conduct clinical assessments and visits to patients in their residences and care facilities.
  • Work under the guidance of registered healthcare professionals to deliver high-quality care.
  • Demonstrate flexibility and strong teamwork skills while managing the demands of a dynamic work environment.
  • Utilize excellent communication and IT skills to engage effectively with patients, caregivers, and multidisciplinary team members.

We welcome applications from experienced healthcare professionals with proven clinical skills and a commitment to patient-centered care. Previous experience in a community healthcare setting is highly desirable but not mandatory.

Qualifications and Experience

Essential Criteria:

  • NVQ Level 3 in Health and Social Care or equivalent.
  • A minimum of two years of experience in a healthcare setting.
