About the Role

The successful candidate will be responsible for providing evidence-based practice for patients with acute and chronic conditions, as well as managing and preventing nursing interventions for our Practice population. You will work collaboratively with our multidisciplinary team to meet the needs of our patients and deliver a healthier community.

Key Responsibilities
  • Deliver high-quality patient care, ensuring the delivery of safe and effective nursing care for our Practice population.
  • Work collaboratively with our multidisciplinary team to meet the needs of our patients and deliver a healthier community.
  • Manage and prevent nursing interventions for our Practice population, including the management of patients with long-term conditions.
  • Provide evidence-based practice for patients presenting with acute and chronic conditions.
  • Work autonomously as a Practitioner, demonstrating critical thinking and skills in clinical decision-making.
Person Specification

We are looking for a candidate with the following qualifications and experience:

  • Registered General Nurse.
  • Current NMC Registration.
  • GCSE Grade A to C in English and Maths.
  • Clinical Supervision and Training Experience.
  • Experience of working within Primary Care.
  • Experience of quality initiatives such as clinical benchmarking.
  • Experience of management of patients with long-term conditions.
Desirable Qualifications and Experience

We would also consider candidates with the following qualifications and experience:

  • Community Nursing Specialist Qualification or Equivalent.
  • Relevant Nursing Degree.
  • Wound Care / Removal of sutures & staples Travel Medicines.
  • Paediatric phlebotomy Insight into public health issues in the local area Awareness of issues within the wider health arena.
