Practice Nurse Specialist

Urban Village Medical Practice is seeking a skilled and dedicated Practice Nurse to join our team. As a Practice Nurse, you will play a vital role in delivering high-quality patient care and contributing to the overall success of our practice.
Key Responsibilities:- Health Promotion and Well-being: Develop and implement health promotion and well-being programs to support patients in maintaining good health and preventing chronic diseases.
- Treatment Planning: Implement and evaluate individual treatment plans for patients with chronic diseases, ensuring effective management and prevention of adverse effects.
- Patient Support: Identify, manage, and support patients at risk of developing long-term conditions, providing guidance and resources to promote healthy lifestyle choices.
- Routine Nursing Care: Provide routine nursing care to patients as required, adhering to clinical-based evidence, NICE guidelines, and the NSF.
- Quality-Based Initiatives: Participate in quality-based local initiatives, such as Manchester Standards, to enhance service delivery and patient care.
- Wound Care and Travel Medicine: Provide wound care and travel medicine services to patients, ensuring effective management and prevention of complications.
- Pathology Services: Request and process pathology results as necessary, ensuring timely and accurate diagnosis and treatment.
- Chronic Disease Clinics: Deliver patient care in chronic disease clinics, referring patients to secondary or specialist care as required.
- Clinical Records: Maintain accurate clinical records in accordance with extant legislation, ensuring confidentiality and data protection.
- Read Codes and Registers: Ensure effective use of read codes and maintain chronic disease registers, supporting data-driven decision-making.
- Well Woman Clinics: Develop and implement well woman clinics, promoting women's health and well-being.
- Chaperoning and Prioritization: Chaperone patients as necessary and prioritize health issues, intervening appropriately to support patient care.
- Clinical Emergencies: Support the team in dealing with clinical emergencies, ensuring timely and effective response.
- Mental Health and Vaccination: Recognize, assess, and refer patients presenting with mental health needs, and implement vaccination programs for adults and children.
- Medicine Support and Liaison: Support patients in the use of prescribed medicines or over-the-counter medicines, and liaise with external services to ensure patient support.
- Delegation and Safeguarding: Delegate clinical responsibilities appropriately, ensuring safe practice and adherence to scope of practice, and support the clinical team with safeguarding matters.
- Substance Abuse and Addictive Behavior: Understand practice and local policies for substance abuse and addictive behavior, referring patients appropriately.
- Health Promotion and Shared Learning: Deliver opportunistic health promotion and participate in shared learning within the practice, enhancing service delivery and patient care.
- Audit Lead: Act as the audit lead, effectively utilizing the audit cycle to drive quality improvement.
- Medicament Storage and Disposal: Monitor and ensure the safe storage, rotation, and disposal of medicaments.
- Nursing Team Support: Support junior members of the nursing team, providing guidance and mentorship as necessary.
- Service Delivery and Patient Care: Participate in local initiatives to enhance service delivery and patient care, supporting the practice's mission and values.
- Shared Learning and Review: Support and participate in shared learning within the practice, continually reviewing clinical practices and responding to national policies and initiatives.
- Root Cause Analysis: Participate in the review of significant and near-miss events, applying a structured approach to root cause analysis (RCA).
