Position Overview: The role of the Patient Flow Coordinator is essential in ensuring effective management of patient discharge processes within the healthcare setting.

Key Responsibilities:

  • Collect comprehensive patient data relevant to their progress and discharge needs.
  • Maintain precise and concise documentation in the clinical database and on the patient tracking system.
  • Participate in daily meetings focused on discharge planning.
  • Facilitate meetings related to patient flow and ensure follow-up on agreed actions.
  • Engage with patients and their families to review discharge plans in accordance with multidisciplinary team (MDT) recommendations.
  • Ensure that all written records are accurate and timely, adhering to Trust policies, and report any discrepancies as necessary.
  • Collaborate with nursing, medical, and pharmacy teams to guarantee that all medications are prescribed and available before discharge.
  • Identify and escalate any obstacles that may hinder timely discharge through the established protocols.
  • Assist all teams in organizing and scheduling patient transport.

Compassionate Care: It is vital to demonstrate empathy and understanding in all interactions with patients and their families.
