About the Role

We are seeking a highly motivated and experienced Health and Justice Navigator to join our team at Midlands Partnership NHS Foundation Trust. As a key member of our team, you will be responsible for providing health and social care support to individuals within the criminal justice system.

Key Responsibilities
  • Provide health and social care assessments and screenings to individuals in custody suites
  • Work closely with police, courts, and other agencies to identify and address health and social care needs
  • Develop and implement care plans to support individuals in achieving their goals
  • Provide advice and information to key decision-makers in the criminal justice system
  • Collaborate with other agencies to ensure seamless transitions and effective communication
Requirements
  • Professional recognized qualification in a relevant field (e.g., nursing, social work, occupational therapy)
  • Knowledge of mental health issues, learning disabilities, and the criminal justice system
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
  • Ability to maintain confidentiality and work with sensitive information
