About the Role

We are seeking a highly motivated and experienced professional to join our team as a Health and Justice Practitioner. This is an exciting opportunity to work in a dynamic and challenging environment, providing high-quality services to individuals in need.

Key Responsibilities
  • Provide compassionate and empathetic support to vulnerable individuals in custody suites, working within the principles of the Care Programme Approach and Community Care Act.
  • Collaborate with other agencies, including the police, courts, and social services, to identify and address the health and social care needs of individuals.
  • Develop and implement effective screening and assessment tools to identify vulnerabilities and provide appropriate referrals to mainstream health and social care services.
  • Work independently and as part of a team, providing advice and guidance to service users, carers, and professionals.
  • Maintain accurate and sensitive records, communicating effectively with individuals, relatives, carers, and the multi-disciplinary team.
  • Promote effective communication and collaboration across teams, agencies, and organisational boundaries to ensure the delivery of an integrated service.
Requirements
  • Registered Mental Health Nurse, Learning Disability Nurse, or General Nurse on the NMC Register.
  • Registered Social Worker.
  • Post-registration experience, evidence of formal courses and training at diploma/degree level.
  • Leadership qualification, qualification in providing supervision, and qualification in coaching.
  • BSc degree in nursing or equivalent level.
  • Evidence of extensive post-registration experience working with clients experiencing mental health-related difficulties.
  • Experience of working with clients who have difficulty engaging with services and have complex mental health needs.
  • Experience of working in a community setting and making decisions autonomously.
