Key Responsibilities
The Pharmaceutical Care Specialist will engage in a variety of patient-centered activities, including:
- Conducting comprehensive medication assessments and managing chronic conditions through expert pharmaceutical knowledge.
- Facilitating seamless transitions of care between healthcare settings, ensuring effective communication and medication adjustments as necessary.
- Assisting patients and healthcare professionals with medication-related inquiries and monitoring.
- Performing detailed medication reviews to effectively manage patients with complex medication regimens.
- Focusing on individuals with multiple health conditions to optimize their treatment plans.
- Providing specialized knowledge in pharmacotherapy while addressing public health and social care needs.
- Utilizing audits and strategic planning to address healthcare disparities.
- Leading initiatives in patient-centered medication optimization.
- Contributing to quality improvement frameworks and enhanced service delivery.
- Implementing structured medication reviews to empower patients in their treatment adherence and self-management.
- Offering pharmaceutical guidance for the development of new healthcare services involving medications.
- Collaborating with general practice teams to ensure compliance with healthcare regulations related to medications.
- Playing a pivotal role in integrating general practice with other healthcare teams to enhance patient outcomes and access to care.
- Building strong relationships with pharmacy professionals across the healthcare network.
- Engaging in clinical aspects of shared care protocols and liaising with specialist and community pharmacists.
Qualifications and Experience
Required
- A minimum of 2 years of experience in a pharmacy setting.
- Proven experience in conducting medication reviews within a primary care context.
- Current registration with the General Pharmaceutical Council.
Preferred
- Over 2 years of experience as a practicing pharmacist.
Educational Background
Required
- Master of Pharmacy or equivalent qualification.
- Certification in independent prescribing.
