Key Responsibilities

The Hospital Palliative Care Team is committed to collaborating with multidisciplinary teams within the hospital to ensure the highest quality of care for our patients and their loved ones. We maintain close partnerships with local hospices and community palliative care services, as well as other network palliative care teams, to provide seamless care for our patients. Our continuous goal is to enhance our services to meet the diverse needs of those we serve.

You will play a vital role, alongside current team members, in advancing specialist palliative care services and fostering excellence in palliative and end-of-life care throughout the organization. You will be part of a multidisciplinary team that includes Clinical Nurse Specialists, Consultants in Palliative Medicine, an End of Life Educator, a Quality Improvement Facilitator, and a Lead Nurse across the organization.

Detailed Job Description and Main Responsibilities

As a registered nurse or allied health professional with extensive specialist experience in palliative and end-of-life care, you must possess: a Non-Medical Prescribing (NMP) qualification (or be willing to pursue it), proficiency in IT, the ability to set your own plans and priorities, and manage your workload to meet objectives and deadlines. You must be an exceptional team player while also having the experience and skills to work independently and make safe, effective decisions in the absence of clinical support. Leadership capabilities are essential for this role, as you will take the lead in various aspects of service development.

You will collaborate with existing nursing and medical staff and will report to the Lead Nurse in Palliative & End of Life Care.

The service operates across multiple sites, and you will be part of our comprehensive seven-day service.
