About the Role

We are seeking a highly skilled Clinical Pharmacist to join our team at Buckshaw Village Surgery Group. As a Clinical Pharmacist, you will play a vital role in the delivery of high-quality patient care and contribute to the development of our medicines management services.

Key Responsibilities
  • Conduct clinical medication reviews to proactively manage patients with complex polypharmacy, focusing on those with multiple co-morbidities.
  • Provide specialist expertise in the use of medicines, addressing both public health and social care needs of patients.
  • Lead on person-centred medicines optimisation, using audits and action planning to tackle inequalities.
  • Contribute to the General Practice quality and outcomes framework (QOF) and enhanced services.
  • Develop and implement new services with medicinal components, providing pharmaceutical advice on treatment pathways and patient information leaflets.
  • Work closely with general practice teams to ensure compliance with CQC standards where medicines are involved.
  • Develop relationships with other pharmacy professionals across Primary Care Networks and the wider health and social care system.
Person Specification

We are looking for a candidate with:

  • Minimum 2 years experience as a pharmacist.
  • Experience of medication reviews relevant in General Practice.
  • Registration with the General Pharmaceutical Council.

Desirable qualifications include:

  • More than 2 years experience as a pharmacist.
  • Master of Pharmacy or equivalent.
  • Independent prescribing.
