Job Summary: We are seeking a highly motivated and compassionate Outreach Support Worker to join our team at Two Saints. As a key member of our outreach team, you will play a vital role in supporting individuals who are sleeping rough in Wokingham borough council.

Main Responsibilities:

  • Identify and support individuals who are rough sleeping in Wokingham borough council, including those with a local connection to the area.
  • Work with individuals to create clear housing plans and support them to access services around health and wellbeing, money, and housing.
  • Support rough sleepers who have been placed into temporary/permanent accommodation to prevent individuals returning to the streets.
  • Carry a caseload of clients and provide an initial assessment to identify client needs and produce person-centered outcome and support plans and risk assessments.
  • Provide personalized support that is flexible, responsive, and supports the client both practically and emotionally.
  • Work alongside other agencies to enable individuals to achieve their goals, including landlords.
  • Maintain good quality records.
  • Work some early mornings, evenings, and occasional weekends to meet the needs of clients and identify new rough sleepers.
  • Participate in bi-monthly street counts.

Requirements:

  • Clear verbal and written English.
  • Good IT and keyboard skills.
  • Polite, assertive manner, ability to self-motivate.
  • Passion for working in a client/customer-facing role.
  • Knowledge of the benefits system and tenancy legislation.
  • Interest and genuine concern for homelessness and related issues.
  • Knowledge of voluntary and statutory agencies.
  • A full driver's license and access to your own vehicle is essential for this role.
