Specialist Nurse in Anticoagulation Therapy

Job Overview
The Clinical Nurse Specialist (CNS) will be an integral part of a nurse-led clinic dedicated to managing comprehensive anticoagulant care. This includes initiating treatment, providing patient education and counseling, managing peri-operative needs, and assessing treatment plans.
As a vital team member, you will collaborate closely with Consultants and fellow Nurse Specialists to facilitate nurse-led clinics for patients undergoing anticoagulant therapy. Strong communication skills are essential, as you will be interacting with patients and their families during potentially stressful situations. You will be responsible for developing personalized treatment plans that align with the latest research and clinical guidelines. A background as a registered general nurse with significant nursing experience, ideally in acute or community settings, is required. Experience in independent practice and management is also advantageous.
Main Responsibilities
The key responsibilities of this role include:
- Conducting reviews for all new patients
- Supervising the discharge process for patients returning home on anticoagulants
- Providing training, education, and counseling for patients on anticoagulants, including vitamin K antagonists, Direct Oral Anticoagulants, and low molecular weight heparin
- Scheduling follow-up appointments to assess patients in the nurse-led service
- Performing INR testing using a Near Patient Testing device
- Managing patients in the anticoagulant clinic, reviewing INR levels, and adjusting warfarin dosages as necessary
- Overseeing anticoagulation management during peri-operative periods
- Reviewing patients who have completed their anticoagulant treatment
- Collaborating with the Lead Haematology Consultant on complex cases
- Actively participating in the prevention of venous thromboembolism and contributing to the organization's risk assessment and thromboprophylaxis strategies
- Assisting in the management and oversight of Band 6 CNSs and administrative personnel
- Supporting the Lead CNS and Haematology Consultant in service development and enhancement
As part of the team, you will work in close partnership with Consultants and existing Nurse Specialists to deliver high-quality care for patients on anticoagulant therapy. Your advanced communication skills will be crucial in addressing the needs of patients and their families during challenging times. You will be expected to take an active role in formulating individualized treatment plans based on current research and clinical guidelines. A solid foundation in nursing, particularly in acute or community care, is essential, along with experience in autonomous practice and management.
