Anticoagulation Nurse Specialist

Job Overview
The Clinical Nurse Specialist (CNS) will be an integral part of a nurse-led clinic dedicated to managing various components of anticoagulant therapy. This includes initiating treatment, providing education and counseling, managing care during surgical procedures, and evaluating or discontinuing treatment as necessary.
As a key member of our team, you will collaborate closely with Consultants and fellow Nurse Specialists to operate nurse-led clinics for patients undergoing anticoagulant therapy. Strong communication skills are essential, as you will interact with patients and their families during potentially stressful situations. You will also be expected to contribute actively to the development of personalized treatment plans that align with the latest research and clinical guidelines. Candidates should possess significant nursing experience, ideally with a background in acute or community care. Experience in independent practice and management is advantageous.
Main Responsibilities
The primary duties of this role include:
- Conducting assessments of all new patients
- Supervising the discharge process for patients returning home on anticoagulants
- Providing training, education, and counseling to patients regarding anticoagulants, including vitamin K antagonists, Direct Oral Anticoagulants, and low molecular weight heparin
- Scheduling follow-up appointments to monitor patients within the nurse-led service
- Performing INR testing using a Near Patient Testing device
- Managing patients in the anticoagulant clinic, reviewing INR results, and adjusting warfarin dosages as needed
- Overseeing anticoagulation management during surgical procedures
- Reviewing patients who have completed their anticoagulant treatment
- Collaborating with the Lead Haematology Consultant on complex cases
- Actively participating in the prevention of venous thromboembolism and contributing to the Trust's risk assessment and thromboprophylaxis strategies
- Assisting in the management and oversight of Band 6 CNSs and administrative personnel
- Supporting the Lead CNS and Haematology Consultant in service enhancement and development
As part of our dedicated Anticoagulant Team, you will have the opportunity to work autonomously while being supported by a professional team of Clinical Nurse Specialists.
Qualifications and Experience
Essential:
- Registered General Nurse (RGN)
- General training in teaching and assessment
Desirable:
- Training in cannulation and venepuncture
Experience:

- Experience in clinical acute or community settings
- Management experience

- Previous experience in anticoagulation
- Experience in service development
- Experience in conducting staff appraisals
Knowledge and Skills:
