Position Overview
The Clinical Nurse Specialist (CNS) will be an integral part of a nurse-led clinic dedicated to managing various facets of anticoagulant therapy. This includes initiating treatment, providing patient education and counseling, managing care during surgical procedures, and assessing treatment effectiveness.
As a key team member, you will collaborate closely with Consultants and fellow Nurse Specialists to facilitate nurse-led clinics for patients undergoing anticoagulant treatment. Exceptional communication skills are essential, as you will be interacting with patients and their families during potentially stressful times. You will also be responsible for developing personalized treatment plans that align with the latest research and clinical guidelines. A background as a registered general nurse with significant clinical experience, particularly in acute or community settings, is preferred. Experience in independent practice and management will be advantageous.
Main Responsibilities
The primary duties of this role include:
- Conducting comprehensive reviews of all new patients.
- Supervising the discharge process for patients transitioning home on anticoagulants.
- Providing training, education, and counseling for patients regarding anticoagulants, including vitamin K antagonists, Direct Oral Anticoagulants, and low molecular weight heparin.
- Scheduling follow-up appointments to monitor patients within the nurse-led service.
- Performing INR testing using Near Patient Testing devices.
- Consulting with patients in the anticoagulant clinic, reviewing INR results, and adjusting warfarin dosages as necessary.
- Managing anticoagulation care during peri-operative periods.
- Reviewing patients who have completed their anticoagulant treatment regimen.
- Collaborating with the Lead Haematology Consultant on complex patient cases.
- Actively participating in initiatives to prevent venous thromboembolism and contribute to the Trust's risk assessment and thromboprophylaxis strategies.
- Overseeing and mentoring Band 6 CNSs and administrative personnel.
- Assisting the Lead CNS and Haematology Consultant in the development and enhancement of services.

Qualifications and Experience
Essential:
- Registered General Nurse (RGN)
- Training in teaching and assessment methodologies
Desirable:
- Training in cannulation and venepuncture
Experience Requirements

- Prior experience in anticoagulation
