This hub serves as the primary access point for individuals aged 18 and above who are facing life-limiting conditions. It is dedicated to coordinating care, facilitating communication, and delivering support to patients, families, and healthcare professionals through a team of nurses proficient in palliative and end-of-life care.

Main Responsibilities

The Palliative Care Specialist will take charge of evaluating, planning, and delivering high-quality, evidence-based nursing care tailored to patients with palliative needs, including those at the end of life. This role also involves providing support to families before and after the death of a loved one, while ensuring that patient dignity, privacy, and independence are upheld.

Detailed Responsibilities

Clinical Duties

  • Operate independently, applying clinical expertise to manage a caseload of palliative patients through holistic assessments and personalized care plans.
  • Respond to patient calls, identifying subtle indicators of distress among patients and families.
  • Assess and prioritize patients based on urgency, ensuring timely interventions for those in critical condition.
  • Facilitate the swift discharge of patients who wish to receive care in their preferred setting, coordinating with relevant services.
  • Advocate for patients with complex needs, collaborating closely with multidisciplinary teams and community resources.
  • Utilize advanced communication skills to discuss sensitive topics such as diagnosis, prognosis, and care planning.
  • Recognize and address safeguarding issues in accordance with relevant legislation.
