Position Overview

The Clinical Nurse Specialist (CNS) will be an integral part of a nurse-led clinic dedicated to managing various components of anticoagulant therapy, including initiating treatment, providing education and counseling, overseeing peri-operative care, and evaluating ongoing treatment.

As a key member of our multidisciplinary team, you will collaborate closely with Consultants and Nurse Specialists to facilitate nurse-led clinics for patients undergoing anticoagulant therapy. Exceptional communication skills are essential, as you will be interacting with patients and their families during potentially stressful situations. You will be expected to actively contribute to the development of personalized treatment plans based on the latest research and clinical guidelines. A background in general nursing, particularly in acute or community settings, is preferred, along with experience in independent practice and management.

Main Responsibilities

The primary responsibilities of this role include:

  • Conducting assessments of all new patients
  • Supervising the discharge process for patients transitioning home on anticoagulants
  • Providing education and counseling to patients regarding anticoagulants, including vitamin K antagonists, Direct Oral Anticoagulants, and low molecular weight heparin
  • Scheduling follow-up appointments to monitor patients within the nurse-led service
  • Performing INR testing using Near Patient Testing devices
  • Evaluating patients in the anticoagulant clinic, reviewing INR results, and adjusting warfarin dosages as necessary
  • Managing anticoagulation for patients undergoing surgical procedures
  • Reviewing patients who have completed their anticoagulant treatment
  • Collaborating with the Lead Haematology Consultant on complex cases
  • Participating in initiatives to prevent venous thromboembolism and contributing to the Trust's risk assessment and thromboprophylaxis strategies
  • Overseeing and mentoring Band 6 CNSs and administrative personnel within the Anticoagulant Team
  • Assisting the Lead CNS and Haematology Consultant in service enhancement and development

As part of our dedicated Anticoagulant Team, you will have the opportunity to work autonomously while being supported by a collaborative group of Clinical Nurse Specialists.

A strong nursing background, particularly in acute or community care, is essential, along with experience in independent practice and management.

Qualifications and Training

  • Registered General Nurse (RGN)
  • General training in teaching and assessment
