About the Role

The Leicestershire County Council is seeking a highly skilled and experienced professional to lead the Home Care Assessment Reablement team. As a Reablement Team Leader, you will play a crucial role in helping individuals regain their independence and live fulfilling lives.

Key Responsibilities
  • Undertake first visits, including risk assessments, to identify the needs of service users.
  • Provide day-to-day line management support to Reablement Support Workers, ensuring they have the necessary skills and knowledge to deliver high-quality care.
  • Conduct Care Act Assessments to identify ongoing eligible needs and develop personalized care plans.
  • Work collaboratively with Occupational Therapists and Physiotherapists to deliver agreed Reablement plans, helping service users achieve their goals.
  • Manage a rolling rota shift pattern, including regular weekends, early, late, and day shifts, and bank holidays.
About You
  • Relevant experience working in adult social care settings, with a good understanding of the Council's Adult Social Care strategies.
  • NVQ 3 or equivalent, with relevant experience working with adults in a social care setting.
  • Ability to use IT applications, including Microsoft Outlook, Word, Excel, and database applications, to identify and record critical information.
  • Strong communication and interpersonal skills, with the ability to negotiate and persuade a wide range of people.
  • Ability to work on own initiative, as part of a team, and effectively liaise with other agencies to deliver integrated services.
  • Flexibility to travel and work from various locations throughout the County, with access to a vehicle and a valid driver's license.
