About the Role

The Leicestershire County Council is seeking a highly skilled and experienced Reablement Team Leader to join their Home Care Assessment Reablement team. As a key member of the team, you will be responsible for providing day-to-day line management support to Reablement Support Workers and undertaking Care Act Assessments where an ongoing eligible need has been identified.

Key Responsibilities
  • Undertake first visits, including risk assessments, to ensure the delivery of high-quality reablement services.
  • Provide day-to-day line management support to Reablement Support Workers, including coaching, mentoring, and performance management.
  • Undertake Care Act Assessments to identify and record critical information, and demonstrate quick thinking and problem-solving skills.
  • Work collaboratively with Occupational Therapists and Physiotherapists to deliver an agreed Reablement plan, helping Service Users identify and achieve their goals.
  • Develop and maintain effective relationships with Service Users, their families, and other stakeholders to ensure the delivery of integrated services and achieve positive outcomes.
About You
  • You will have relevant experience working in adult social care settings and a good understanding of the Council's Adult Social Care strategies.
  • You will hold an NVQ 3 or equivalent and relevant experience working with adults in a social care setting is essential.
  • You will have the ability to use Information Technology applications, including Microsoft Outlook, Word, Excel, and database applications.
  • You will be able to communicate clearly and accurately, both orally and in writing, with a wide range of people, including representatives from other agencies.
  • You will be able to work on your own initiative and as part of the team, effectively liaising and working in partnership with other agencies to ensure the delivery of integrated services and achieve positive outcomes.
Requirements
  • You will need access to a vehicle and a driver's license.
  • You will have a flexible approach to work and be willing to travel and work from various locations throughout the County.
  • You will have an Enhanced DBS and be able to work in a fast-paced environment.
