About the Role

The HART service helps individuals regain their independence by providing reablement support. This short-term support enables people to become as independent as possible by giving them the opportunity, motivation, and confidence to relearn or regain skills for daily living that may have been lost due to illness, accident, or disability.

Key Responsibilities

As a Reablement Team Leader, you will be responsible for undertaking first visits, including risk assessments, providing day-to-day line management support to Reablement Support Workers, and undertaking a Care Act Assessment where an ongoing eligible need has been identified.

Working with the Team

You will work closely with Occupational Therapists and Physiotherapists to help deliver an agreed Reablement plan, enabling Service Users to identify and achieve their goals relating to social care tasks such as washing and dressing, showering, meal preparation, and engaging in community activities.

About You

  • Relevant experience working in adult social care settings and a good understanding of the Council's Adult Social Care strategies.
  • NVQ 3 or equivalent and relevant experience working with adults in a social care setting is essential.
  • Ability to use Information Technology applications, including Microsoft Outlook, Word, Excel, and database applications.
  • Ability to prioritise and manage your workload, meeting deadlines.
  • Ability to communicate clearly and accurately with a wide range of people, including representatives from other agencies.
  • Working on your own initiative and as part of the team, effectively liaising and working in partnership with other agencies to ensure the delivery of integrated services and achieve positive outcomes for Service Users.
