About the Role
Job Summary
The Leicestershire County Council is seeking a highly skilled and experienced Reablement Team Leader to join our Home Care Assessment Reablement team. As a key member of our team, you will be responsible for leading the delivery of reablement services to individuals in their own homes, promoting independence and well-being.
Key Responsibilities
- Lead the Reablement team in the delivery of high-quality reablement services, ensuring that individuals receive the support they need to regain their independence.
- Conduct comprehensive assessments to identify individuals' needs and develop personalized reablement plans.
- Provide day-to-day line management support to Reablement Support Workers, ensuring that they have the necessary skills and knowledge to deliver effective reablement services.
- Work closely with Occupational Therapists and Physiotherapists to deliver integrated reablement services, promoting the health and well-being of individuals.
- Develop and maintain strong relationships with stakeholders, including health and social care professionals, to ensure seamless service delivery.
- Monitor and evaluate the effectiveness of reablement services, identifying areas for improvement and implementing changes as necessary.
Requirements
- Relevant experience working in adult social care settings, with a good understanding of the Council's Adult Social Care strategies.
- NVQ 3 or equivalent qualification in a relevant field, with experience working with adults in a social care setting.
- Ability to use Information Technology applications, including Microsoft Outlook, Word, Excel, and database applications.
- Excellent communication and interpersonal skills, with the ability to work effectively with a wide range of people, including representatives from other agencies.
- Ability to work on own initiative and as part of a team, with a flexible approach to work and a willingness to travel to various locations throughout the County.
