The post holder will work as part of a multi-disciplinary team to support the delivery of recovery care plans for clients with a range of mental health presentations in the community and facilitating discharges from hospital. A key role will be for the post holder to be part of the team supporting our wards to enable safe administration of Electroconvulsive Therapy (ECT).

The post holder will work intensively with individuals to support them in their homes, preventing hospital admissions and facilitating hospital discharges. They will also have part of their time planned for specific activities, including linking with wards to support patients to go out while they are inpatients, supporting the assessment of carers as part of a Devon Memory Service assessment, and supporting the OPMH functional wards to ensure safe transfer and administration of ECT to patients while they are inpatients.

Whether seeing patients in the community or as part of another role, the post holder will be expected to work to a clearly defined personal recovery plan outlining the role for the post holder with that client and the range of interventions required.

Main Duties of the Job
  1. Provide support to older adults in their own homes, delivering practical help and advice where possible.
  2. Consider community support available and link in where possible.
  3. Provide support in escorting individuals to ECT.
  4. The post holder will be expected to support at the Devon Memory Service - training will be offered.
  5. The post holder will need to hold a UK driving license.
