Key Responsibilities:- Deliver essential practical, personal, and social assistance to young individuals aged 16 to 18 transitioning from care.
- Accurately document reports and develop comprehensive support plans.
- Assess potential risks and work collaboratively with the team to ensure a safe environment.
- Encourage young people in their educational, training, or employment pursuits.
- Uphold a high standard of appearance in placements.
- Facilitate community engagement activities and key work sessions.
- Coordinate with external agencies and professionals for comprehensive support.
- Establish trusting relationships with young individuals and stakeholders.
- Experience in working with youth or managing challenging behaviors.
- Strong organizational capabilities and proficiency in IT.
- Empathy and insight into the needs of young adults transitioning from care.
- Ability to maintain professional boundaries effectively.
- Flexibility to work on a rota basis, including sleep-in shifts.
- Valid UK driving license.
- Eligibility to work in the UK.
- GCSE or equivalent qualifications preferred.
- NVQ Level 3 in Health & Social Care is desirable.
