Key Responsibilities:- Deliver practical, personal, and social assistance to young individuals aged 16 to 18 transitioning from care.
- Accurately complete documentation and support plans.
- Assess risks and collaborate with the team to ensure a safe environment.
- Assist young adults in their educational, training, or employment pursuits.
- Uphold a high standard of living environment.
- Coordinate community engagement activities and individual support sessions.
- Engage with external agencies and professionals as needed.
- Establish trusting relationships with young individuals and stakeholders.
- Experience in working with youth or managing challenging behaviors.
- Strong organizational capabilities and proficiency in IT.
- Empathy and insight into the needs of young adults leaving care.
- Ability to maintain professional boundaries effectively.
- Flexibility to work on a rota basis and participate in sleep-in shifts.
- Full UK driving license is required.
- Eligibility to work in the UK.
- GCSE or equivalent qualifications are preferred.
- NVQ Level 3 in Health & Social Care is advantageous.
