We are seeking a skilled and compassionate Adult Community Inclusion Support Worker to join our team at Devon Partnership NHS Trust. As a key member of our multidisciplinary team, you will play a vital role in supporting individuals with mental health needs in their own homes and communities.
Job Summary:The successful candidate will work under the supervision of mental health practitioners to deliver recovery care plans for clients with a range of mental health presentations. This will involve working across agencies, including Primary Care, Complex Care Teams, and Third sector organizations.
Main Responsibilities:- Work intensively with individuals to support them in their homes, preventing hospital admission and facilitating hospital discharge.
- Monitor mental health through observations and report changes to qualified staff to ensure the care plan remains up-to-date.
- Assist individuals in achieving their recovery goals, promoting independence and well-being.
- Communicate effectively with individuals, their carers, and other professionals to ensure seamless care coordination.
- Participate in regular supervision and training to maintain professional development and adherence to Trust policies.
- Demonstrate an ability to make and sustain professional therapeutic relationships.
- Good communication and listening skills.
- Ability to work with individuals in a group and individual basis.
- Able to work in a team effectively.
- Demonstrate an ability and awareness to maintain personal boundaries.
- NVQ level 3, QCF Level 3 in Health and Social Care or equivalent qualification.
- Previous employment or experience working with individuals in a caring situation or previous experience working in a different role within mental health services.
- Previous care experience with individuals who have mental health difficulties.
