About the Role

We are seeking a highly motivated and compassionate Community Support Worker to join our team at Devon Partnership NHS Trust. As a Community Support Worker, you will play a vital role in supporting individuals with mental health needs in their own homes and communities.

Key Responsibilities

  • Work closely with mental health practitioners to deliver recovery care plans for clients with a range of mental health presentations.
  • Provide intensive support to individuals in their own homes, preventing hospital admission and facilitating hospital discharge.
  • Monitor mental health through observations and report changes to qualified staff to ensure the care plan is kept fully up to date.
  • Work with individuals and their carers to achieve the objectives of the care plan.
  • Encourage social and physical activities as identified within the care plan.

Requirements

  • Demonstrate an ability to make and sustain professional therapeutic relationships.
  • Good communication and listening skills.
  • Ability to work with people in a group and individual basis.
  • Good written communication.
  • Able to work in a team effectively.
