Typical Responsibilities of a Support Coordinator

  • Developing personalized support plans in collaboration with clients.
  • Assisting clients with essential daily living tasks such as budgeting, tenancy maintenance, and transitioning to independent housing when possible.
  • Conducting risk assessments, formulating support strategies, setting goals, and performing regular evaluations.
  • Providing guidance and directing clients to external services to foster their independence.
  • Engaging in welfare checks one day and assisting with room preparations the next.
  • Enjoying meaningful conversations and collaboration with colleagues without the pressure of rushing.

Your Qualifications

  • A strong passion for empowering clients to lead fulfilling lives, with a collaborative approach and attention to detail.
  • Experience in creating individualized support plans and guiding colleagues in their implementation.
  • Background in coordinating and evaluating client referrals.
  • Experience supporting vulnerable individuals, coupled with a positive attitude and a desire to learn and make a difference.
  • Ability to work independently, maintain composure under pressure, and exhibit resilience.
  • A vehicle insured for business use is required, and mileage will be reimbursed.

Job Details

  • Flexible working hours, with an openness to discuss your preferred work pattern.
  • While we prioritize flexibility, we must also meet the needs of our clients, which may require occasional adjustments to your schedule.
  • Work will follow a rota pattern, including one weekend every three weeks.
  • Although you will work independently, regular training and team meetings will be held.
  • Proficiency in technology for creating and updating support plans, completing online training, and collaborating with colleagues is essential.
  • An Enhanced DBS check will be required, and we will cover the cost.
