ROLE DESCRIPTION:


As a Client Support Coordinator, you will serve as a vital link among our Clients, Support Workers, and Package Managers. We seek an individual who is self-assured, skilled, and prepared to embrace challenges.

You will need to showcase a proven track record in coordinating personnel and systems while functioning collaboratively within a team to assist Package Managers effectively.

Your responsibilities will include ensuring that all communications and documentation essential for our clients' rehabilitation are executed efficiently.

PRIMARY RESPONSIBILITIES:

  • Maintain weekly communication with available Support Workers (SW)
  • Update the internal database with SW availability
  • Develop schedules based on SW availability and confirm their participation
  • Manage daily, weekly, monthly, and emergency shift coverage, updating the internal system as necessary
  • Assist Package Managers in gathering essential documentation for client visits and referrals
  • Support and monitor the use of I-connect
  • Coordinate with support workers regarding MACH (Daily reports) forms, timesheets, and I-connect
  • Aid Package Managers in reviews and maintaining statutory documentation (Care Plan, Risk Assessment, Medication Support Plan, Environmental Risk Assessment)
  • Help Package Managers organize and participate in staff meetings, Multi-Disciplinary Team meetings, and Package Managers meetings
  • Assist Package Managers in creating, reviewing, and amending MACH forms using the online system
  • Conduct regular client interactions, reviews, updates, and general spot checks, including assisting with Medication Competency Assessments
  • Complete quarterly supervisions with SWs, assessing personal development in accordance with company policy
  • Collaborate with other departments to facilitate recruitment for packages
  • Participate regularly in the on-call rota
  • Distribute PPE as needed
  • Schedule Meet & Greets, including checking SW availability and onboarding new staff
  • Arrange shadow shifts, coordinating with all parties to align availability
  • Ensure documentation is current and accurate
  • Attend to on-call responsibilities as required

ADDITIONAL RESPONSIBILITIES:
  • Engage in the supervision and appraisal system
  • Undertake any other additional duties as assigned by management

TRAINING:
