At Royal Trinity Hospice, we are embarking on an exciting journey as we unveil our new community services, forming a vital part of our clinical hub. These innovative services have been designed to enhance the coordination of care for patients, addressing the evolving needs in palliative and end-of-life care within our community while maximizing our responsiveness and flexibility.

We are seeking a dedicated, experienced, and highly motivated professional to join our First Contact Team as a Palliative Care Triage Nurse Specialist.

In this role, you will serve as the initial clinical point of contact for patients following their referral. Your responsibilities will include identifying and addressing immediate specialist palliative care needs through direct communication with patients, caregivers, and other healthcare professionals involved in their care.

Key Responsibilities
  • Lead the process of receiving and managing referrals directed to the hospice.
  • Triage all incoming referrals, evaluating patients' reported symptoms to ascertain the appropriate level of care necessary to meet their specialist palliative care requirements safely.
  • Utilize established resources and guidelines to effectively assess, prioritize, advise, schedule patient care management, or refer patients to other suitable health and social care agencies when needed.
  • Ensure a timely, responsive, supportive, and effective service for all stakeholders involved in patient referrals.
  • Oversee all aspects of the hospice referral system, identifying appropriateness, urgency, and priority levels.
  • Collaborate with internal and external partners to facilitate the coordination and management of patients referred to the hospice, ensuring care is tailored to meet the individual and dynamic needs of patients and their families.
  • Use assessments and interventions as opportunities to train, coach, and mentor clinical staff and visitors regarding specialist palliative care practices.
Skills and Experience Required
  • Registered Nurse with a bachelor's degree in palliative/end-of-life care or a related field, along with extensive experience in a specialist palliative care setting.
  • Advanced assessment and critical thinking skills.
  • Demonstrated knowledge of disease trajectories and treatment effects.
  • Exceptional communication skills.
  • A flexible attitude and enthusiasm for the role.
  • Ability to work independently as well as collaboratively within a team.
  • The successful candidate should possess a valid UK driving license for ease of travel across our service area.
