At Royal Trinity Hospice, we are excited to announce the launch of our innovative community services, forming a new clinical hub designed to enhance patient care coordination. These services aim to address the evolving needs of our local population in palliative and end-of-life care, ensuring we provide flexible and responsive support.

We seek a dedicated, experienced, and motivated individual to join our First Contact Team as a Palliative Care Triage Nurse Specialist.

In this role, you will serve as the initial clinical point of contact for patients following their referral. Your responsibilities will include identifying and addressing immediate specialist palliative care needs through direct communication with patients, caregivers, and other healthcare professionals.

Key Responsibilities
  • Lead the management and reception of referrals directed to the hospice.
  • Triage incoming referrals by evaluating patients' reported symptoms to determine the necessary level of care for their specialist palliative needs.
  • Utilize established resources and guidelines to effectively assess, prioritize, advise, and manage patient care or refer them to appropriate health and social care agencies when necessary.
  • Ensure a timely, responsive, and effective service for all stakeholders involved in patient referrals.
  • Oversee all aspects of the hospice referral system, assessing appropriateness, urgency, and priority levels.
  • Collaborate with internal and external partners to support the coordination and management of referred patients, ensuring care is tailored to meet their individual and dynamic needs.
  • Use assessments and interventions as opportunities to educate, mentor, and coach clinical staff and visitors regarding specialist palliative care practices.
Skills and Experience Required
  • Registered Nurse with a bachelor's degree in palliative/end-of-life care or a related field, along with extensive experience in a specialist palliative care setting.
  • Advanced assessment and critical thinking skills.
  • Strong knowledge of disease trajectories and treatment effects.
  • Excellent communication skills.
  • A flexible attitude and enthusiasm for working in a dynamic environment.
  • Ability to work independently as well as collaboratively within a team.
  • The successful candidate should possess a valid UK driving license for ease of travel across our service area.
