About the Role

We are seeking a compassionate and skilled Clinical Nurse Specialist to join our Community Palliative Care Team at St Christopher's Hospice. As an Associate Clinical Nurse Specialist, you will play a vital role in supporting people with life-limiting illnesses and their families, providing expert care and guidance to ensure a dignified and peaceful end of life.

Key Responsibilities
  • Provide expert care and support to patients and their families, including symptom control, social, emotional, and spiritual care.
  • Work collaboratively with the interdisciplinary team to develop and implement individualized care plans.
  • Conduct telephone and video triage, visiting patients in their homes and providing advice and support to primary healthcare teams and hospital staff.
  • Develop and maintain relationships with local healthcare providers and community organizations to ensure seamless care coordination.
  • Participate in education and training programs to enhance skills and knowledge in palliative care.
Requirements
  • Registered Nurse or Paramedic with experience in end-of-life care or a willingness to develop skills in palliative care.
  • Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team.
  • Ability to work flexibly, including evenings and weekends, to meet the needs of patients and families.
  • Knowledge of palliative care principles and practices, with a commitment to delivering high-quality care.
