We seek a dedicated, skilled, and passionate individual to fill the role of Palliative Care Triage Nurse Specialist within our First Contact Team.

In this position, you will serve as the initial clinical contact for patients following their referral. Your role will involve identifying and addressing immediate specialist palliative care needs through direct communication with patients, caregivers, and other healthcare professionals.

Key Responsibilities
  • Lead the reception and management of referrals directed to the hospice.
  • Triage incoming referrals by evaluating patients' reported symptoms to ascertain the necessary level of care to effectively address their specialist palliative care needs.
  • Utilize established resources and guidelines to assess, prioritize, advise, schedule patient care management, or refer patients to appropriate health and social care agencies when necessary.
  • Ensure a timely, responsive, and effective service for all stakeholders involved in patient referrals.
  • Oversee all aspects of the hospice referral system, determining appropriateness, urgency, and priority levels.
  • Collaborate with internal and external partners to facilitate the coordination and management of patients referred to the hospice, ensuring care is tailored to meet the individual and dynamic needs of patients and their families.
  • Use assessments and interventions as opportunities to train, coach, and mentor clinical staff and visitors regarding specialist palliative care practices.
Skills and Experience Required
  • Registered Nurse with a bachelor's degree in palliative/end-of-life care or a related field, along with extensive experience in a specialist palliative care setting.
  • Advanced assessment capabilities.
  • Strong critical thinking skills.
  • Demonstrated knowledge of disease trajectories and treatment impacts.
  • Excellent communication skills.
  • A positive and adaptable approach to work.
  • Ability to work independently as well as collaboratively within a team.
  • The successful candidate should possess a valid UK driving license for ease of travel across our service area.
