Job Title: Clinical Nurse Specialist - Triage

We are seeking an experienced and skilled Clinical Nurse Specialist to join our team in a key role that requires strong clinical expertise and excellent communication skills.

About the Role

This is an exciting opportunity to work in a dynamic and supportive environment, where you will be responsible for receiving and managing referrals made to our hospice. You will work closely with patients, carers, and other healthcare professionals to identify and respond to the immediate specialist palliative care needs of patients.

Key Responsibilities
  • Receive and manage referrals made to the hospice, using your clinical expertise to assess and prioritize patient needs.
  • Work collaboratively with internal and external partners to support the coordination and management of patients referred to the hospice.
  • Use your advanced assessment skills to identify and respond to the immediate specialist palliative care needs of patients.
  • Develop and implement effective care plans that meet the individualized and dynamic needs of patients and their carers.
  • Provide education and training to clinical staff and visitors to the hospice on specialist palliative care practice.
Requirements
  • Registered Nurse with bachelor's degree level training in palliative/end of life care or related subject and extensive experience working within a specialist palliative care field.
  • Advanced assessment skills and critical thinking skills.
  • Ability to work autonomously and as part of a team.
  • Excellent communication and interpersonal skills.
