Job Summary

We are seeking an experienced and highly motivated Clinical Nurse Specialist to join our team at Royal Trinity Hospice. As a key member of our First Contact team, you will play a vital role in providing specialist palliative care to patients and their families.

Key Responsibilities
  • Referral Management: Receive and manage referrals made to the hospice, ensuring timely and effective assessment and prioritization of patient needs.
  • Triage and Assessment: Utilize advanced assessment skills to determine the appropriate level of care required for patients, considering their stated symptoms and needs.
  • Care Coordination: Work collaboratively with internal and external partners to coordinate and manage patient care, ensuring individualized and dynamic needs are met.
  • Training and Development: Use assessments and interventions as opportunities to train, coach, and mentor clinical staff and visitors to the hospice about specialist palliative care practice.
Requirements
  • Education and Experience: Registered Nurse with bachelor's degree level training in palliative/end of life care or related subject, and extensive experience working within a specialist palliative care field.
  • Skills and Qualities: Advanced assessment skills, critical thinking skills, ability to demonstrate knowledge of disease trajectories and treatment effects, advanced communication skills, enthusiasm, and a flexible attitude to working.
